4. Clinical particulars
4.1 Indications for treatment
How to deal with sudden pain in adults on opioid therapy for long-term cancer pain. Breakthrough pain is a transient exacerbation of otherwise controlled chronic background pain.
4.2 Dosage and administration method
Buy Abstral Sublingual 100MG, which should only be administered to patients who are considered tolerant to their opioid therapy for persistent cancer pain. Patients can be considered opioid tolerant if they take at least 60 mg of oral morphine daily, at least 25 micrograms of transdermal fentanyl per hour, at least 30 mg of oxycodone daily, at least 8 mg of oral hydromorphone daily, or an equianalgesic dose of another opioid for a week or longer.
How the medicine is given:
Abstral Sublingual 100MG tablets should be put in the deepest part of the tongue. Abstral sublingual tablets shouldn’t be swallowed. Instead, they should be left in the sublingual cavity to dissolve completely without chewing or sucking. Patients should not eat or drink anything until the sublingual tablet has dissolved completely.
Before taking Abstral, patients with dry mouths can moisten the buccal mucosa with water.
Adjusting the dose:
The object of dose titration is to identify an optimal maintenance dose for ongoing treatment of breakthrough pain episodes. This optimal dose should provide adequate analgesia with acceptable adverse reactions.
The best amount of Abstral for each patient will be found by slowly increasing the dose. Several doses are available for use during the dose titration phase. The first dose of Abstral should be 100 micrograms. If more is needed, the dose can be increased through the range of strengths.
Patients should be carefully monitored until an optimal dose is reached.
Because of how they are absorbed differently, switching from other fentanyl-containing products to Abstral cannot be done at a 1:1 ratio. If patients are switched from another fentanyl-containing product, a new dose titration with Abstral is required.
For titration, the following dose schedule is suggested. However, the doctor should always take into account the patient’s clinical needs, age, and other illnesses.
All patients must start treatment with a single sublingual tablet of 100 micrograms. If the pain isn’t gone after 15 to 30 minutes of taking a single 100-microgram sublingual tablet, a second 100-microgram sublingual tablet may be given. If the pain isn’t relieved enough 15–30 minutes after the first dose, the next time the pain comes back, the dose should be increased to the next highest tablet strength (Refer to figure below).
Dose escalation should continue stepwise until adequate analgesia with tolerable adverse reactions is achieved. At doses of 400 micrograms or more, the strength of the second sublingual tablet should be raised from 100 micrograms to 200 micrograms. The schedule below shows how this works. During this “titration” phase, you shouldn’t give more than two (2) doses for a single episode of pain.
